New Directions is a community-based case management program for individuals with complex medical and psychosocial needs, many of whom are chronically homeless. Our intensive case management model and “whatever it takes” philosophy have proven to be effective in reducing the frequency of emergency room visits and hospital admissions, and providing overall improvement to quality of life for our clients. Referrals to the New Directions program come directly from partner agencies.
New Directions was established in 2002 by the Hospital Council of Northern and Central California as an innovative project that would connect frequent emergency room users with appropriate medical care and social services to improve patient outcomes and reduce the number of avoidable visits to hospitals. In July 2013, New Directions began a new partnership with Peninsula Healthcare Connection, adding an additional layer of programming to the organization
